Please print and fill out the Medical Information Form and bring it with you to your appointment or you may fill out an Online Form Here.

In addition, please bring:

• Glasses and/or sunglasses that you currently wear

• Your contact lens boxes, if you wear contacts

• Your current health and vision insurance cards – we’ll make copies for
  your chart (if you have questions about your coverage, please call us)

• A list of any medications you are taking
